You'd think Windows would have some internal management of registry fragmentation, wouldn't you. This is likely why some old Windows installs never feel snappy no matter what you do and need to be wiped.

I use the disk defrag of theirs more than anything. Great program that thing is. Ill use their registry defrag from time to time.

Usually with most slow computers i just format anyway or at least recommend it for 2 reasons.
1) if youre trying to scan to remove viruses/malware or what have you, a format and a reinstall is usually quicker because of the speed of the computer or lack thereof.
2) It's more money in my pocket.

1 Absolutely. If I ever detect malware, I don't bother trying to disinfect it, I simply reformat and reinstall, or reimage if there is one. Otherwise, one can never be sure of having fully cleared out the virus, or what hidden damage has been done to Windows.

2 This is of course, the best reason of all.
