I have yet to clean a single graphics card and only had to replace one out of warranty HSF that failed. There are really only two reasons why a card would need to be cleaned when mounted upside down:
1) The slits of the heatsink are too close together causing dust particles to get caught and thus, build up into a large mass over time.
2) The heatsink itself has become attractive to dust (static electricity) because it isn't properly grounded.
The top of the card can still get dust on it but, unless you are overclocking, it is not critical to keep it free of dust.
