You've got the first generation of 9600 GT, they suffered a hardware related problem which cause conflicts with the motherboards chipset. Unless you're prepared to change motherboards the only way to solve it is to RMA the card. Dont waste your time trying to solve it, its a common problem I've experienced it before too and was forced to change motherboards. There are forums with hundreds of people with the same issues with the 9600 GT. It isn't driver related and it isn't your fault, just replace the card.
If it isn't too late to return the card pick up a a different card which doesn't have issues. (happens to be faster too)
Nvidia 9800 GTX+ for about $150
ATI 4830 for about $99
ATI 4850 for about $150

Its working fine but for how long? Hopefully it is only driver related but I doubt it!
