I haven't tried tweaking any settings beyond my extensive texture mods but I still average 45 FPS with 30 in Whiterun even with it being my most heavily modded area. If your card is a 1GB 6870 then it would definitely be v-ram causing the slowdowns I haven't bothered to check how much v-ram I am using with my mods but I am sure it would not be pretty :/
