Normally I'd be outside, and some guy would be following (he'd be a friendly guy) and follow me inside.

They do, at least in my case. I get caves and places marked as "cleared" but when I revisit them there'd be all the bandits there. I wouldn't complain though, more bandits, more necromancers, more killings and more gold . . . mwahhaahhaha
Oh and I also found a treasure map. And it actually led me to real treasure. That was good
Since mine was marked IV and later I got another one marked VI then there are plenty more to explore. . cool
