I've encountered absolutely the same problem at my father's pc. However in my case W8 recovery fixed that issue and it never came back. No idea what it was but my wild guess it was because of HDD.

I wish it was a simple fix, but it wasn't. I even tried refreshing the OS, that failed, and it reverted back to the way it was (thankfully).
