Help identifying a surface mount capacitor

New to the forum and I have a question I hope someone out there can help me out with.

If anyone could identify this capacitor from a nvidia quadro nvs 450 and where I can get it that would be great!

The top line is the case number, the second line is series, third is capacitance and fourth voltage.
