Core 2 Duo e8400

The problem is shown in the image, sometimes playing a video crashes the output it's on. As you can see, the other output is just fine and behaves normally. Sometimes the ... thing dissapears after I force close whatever is playing the video, the time from when the image is taken it dissapeared when I changed the resolution for that monitor.
It used to happen a lot before I reinstalled Windows, and it was doing fine for a bit but it still happens sometimes. It generally only does it when closing flash videos, turning off full screen for flash videos, and once it happened when I opened a flash player link in a new window and the video started playing automatically. Once it happened when I stopped a video playing in VLC (mp4 iirc) and once it happened when going out of full screen in Netflix.
It doesn't matter what I run the GPU at, currently it's downclocked and downvolted.
It's stable in games, this only happens when it plays videos. I'm on the latest WHQsomething drivers.
