Repaste would be the first thing I would do. Looking at YouTube videos seems the cooling fins can come away and leave the baseplate/fan in place so no need for new thermal pads.

Hope its on a new game engine, like the idea of all fallout games but the game engine is just so clunky I can't play them without it distracting me to the point I can't enjoy it

Have a play with nvidia fast sync as well as nvidia control panel v-sync vs in game v-sync. Also triple buffering and render ahead limits can help. Not all games let you adjust this and I can't remember what's in the nvidia control panel.

Have a look for a small macro keyboard, can program it to do what you like then.
