Hello I plugged the Zotac 550 Ti in to my old M2N4-SLI Motherboard and when it booted up my monitor screen was covered in hundreds of horizontal lines roughly a 1cm long and ranging in colour from white to teal to blue. So I plugged back in my old 9800 GTX+ which works fine and waited till I got a new motherboard cause i wanted to get one anyway. So I got a M5A97 and it is doing the same thing. Is the video card completely shot or could it be something else? I got it about 3 weeks ago, so far only tried it with the two motherboards, has not worked once yet.

Its bricked. You pretty much did all testing wise and came to the same conclusion on 2 boards. And if you could power a 9800gtx, its not psu related.

Please fill in your specs in the control panel so we can see what you're working with. What's the PSU model and the hardware it's powering?
