Software A lot.
Benchmark Scores Its fast. Enough.
Sounds like a board component failed on the card, I would suggest a RMA if its still within warranty period, if not perhaps take the card apart and see if you find any damaged areas, take pictures and upload them here.

Benchmark Scores Faster than yours... I'd bet on it. :)
Sounds like its toast... do you have another video card around to test to make sure it isn't the board? Seems like when it hits 3D/drivers are installed it just borks.. not good.

If the card has dual BIOS try switching to the other BIOS. I know. It doesn't make much sense that it would be the problem. But there's not a whole lot making very much sense here. Anyway...it can't hurt.

Do you have multi monitor? if so try unplugging one before you install drivers, though I had a similar issue myself with a previous 290x and the card was dead..

It could be a lot of things. But I don't think it has anything to do with AMD display drivers. I think it's most likely some kind of hardware problem. Probably the card itself is defective. The best way to find out would be to install it in a different system and see if the problem persists. If the card works fine in a different system then the card most likely isn't the problem. Or you could try installing a known working card in the same system you're having the problem with. If the known working card works without problems then it's probably not a hardware problem with the system.

Random thought but check your card's clock speeds versus reference. I had a card that factory overclock wasn't stable, reference was.

Replace thermal compound on the card, better power supply.

@AsRock had a weird issue like that.

Based on one of your posts I'm going to guess that you purchased this secondhand, from a private seller. Based on yet another one of your posts ,it seems you've owned this card anywhere from 3 to 6 or more months. Based on the above two factors I'm going to guess you're stuck with this card ,so it's something you have to either resolve or replace hopefully the former over the latter.

Hopefully one of the suggestions posted above will work toward this goal.... make sure you're at either stock frequencies or maybe even a little lower, also verify thermal pads and thermal paste are all in good shape. If you have a second bios ,switch to that ..hopefully these tips will resolve your problem

while you may have solved the current problem i suspect this Card is slowly dieing and sooner or later its going to have the 3rd time the charm Killer heart Attack. ( and off to silicon heaven/hell it goes).

Start Saving and looking for the inevitable Replacement

Try the card in another computer and if the problem follows the card, it's the card.

I got same problem 6 years ago, i was cheated too when i bought used EVGA GTX 260 black edition, for 4 months card was working correctly, than problems started, every day was worse and worse and died leaving me without picture at all
I have tried to bake in oven which helped a bit but problems has return

What i wanted to say is: you bought repaired card from some1 who repaired it instead to trow it into garbage
Next time better buy new card or used card from trusted source

Wtf....

The card is defective, baking might get some more life out of it but the fact of the matter is you need a new gpu

Not much other than just simply running games or benchmarks.

A Good Visual Inspection followed by
Not much other than just simply running games or benchmarks.

If it passes the above then its probably Ok if not
Then it needs to be Returned to the Vendor ( YOU ONLY GET CARDS THAT CAN BE RETURNED).

A broken card is a broken card , doesn't matter it is AMD or Nvidia. This is not manufacturer specific. Although at this point I would start to suspect it's a bad PCIe slot.
