Can you identify what card model from the pcb & chip?

So my friend gave me this old graphic card. The fan is broken so I decided to take out the cooler. I wanted to buy a replacement cooler which cost 8 dollars but I want to make sure is it worth it or not (used old cards are cheap).
The problem is I don't know what model is this and I don't have a desktop pc I can use to gpu-z-it atm.
From the look on the back of the card, it was purchased around 2008, could be 3000/4000 series. It has cf connector and no additional power connector which could be 3750/70 or 4650/4670.
The old cooler has no branding and the propeller was gone by the time I received it.
Can one identify the card model from the pcb without any kind of brand printed on it?
The chip says:

Can someone tell me what card is this and how do you find out (without putting it in a pc)?

Looks like an hd 6450, I just put "ati 0834" into Google and a lot of suggested searches were for the pegatron 6450.

After some research, the PCB was used for the 4670 too so I'm not sure after all :p

Thanks, so this might be 4650/4670. The boards shown on both link look very similar. 4650 cost around 18 USD around here so.. i think it might be worth it to replace the cooler.

Thanks, so this might be 4650/4670. The boards shown on both link look very similar. 4650 cost around 18 USD around here so.. i think it might be worth it to replace the cooler.
The pcb code thing is very helpful :)

It's a 2008 card tho so there's not much use for it except for an old school build. Current integrated graphics are more powerful.

In my case, I'm going to build a pc with ryzen, which doesn't have an integrated gpu so 4670 for 8 dollar (just for replacement cooler) is quite a good deal while I wait for next gen geforce this year (rumor). Until then, can't do decent gaming yet but should be enough for my daily tasks.

Just an update,
I managed to install replacement cooler which is Deepcool V95 into this card. Had to cut a portion of its mounting plate to mount properly. There's a fan header on top of the card that gets in the way so I decided to destroy the plastic frame and solder the connector directly to fan. It has successfully booted into bios but I haven't use it yet in windows, so I can't give confirmation whether this is 4650 or 4670 yet.

The thing is, this card doesn't turn on if you don't plug in a fan on its fan header I don't know why. Placing jumper won't work it has to be a 5V fan or it wont work. So I grab a random small 5V fan and solder it. Now I have to deal with the dangling 50mm fan I have to tape somewhere within the case until I find a 3 pin connector female header (I don't want to cut the 3 pin male connector from the original fan)
