I'm currently working on a very similar issue (DR site for loss of service due to hardware/people failure) but unfortunately I have to go into a meeting now, but I will send you a PM later and share our DR plans.

In short.

A plan for each scenario (redundant host/server, second in command to take over etc)
A procedure for each scenario ( who invokes DR, and the steps to make it happen)
A procedure for returning from DR to normal service.
A battle box (this is where you keep the plan, passwords, contact details etc)

and most importantly a test plan. ( we do 2 DR tests a year where we intentionally fail our services), One test is very limited involving a handful of people, the second is company wide (45k people across 400 locations). Its amazing the holes you find in your plans when you do something like this
