What is happening with my GPU?

Out of nowhere GTA V randomly starts to get laggy and laggy over a short period of days I wanted to know if there was anything up with my GPU I use AMD's own system monitor and this is what it shows me!
Help would be much appreciated.

Notes:
This shot was taken while playing GTA V
I don't have onboard graphics.
