GPU Issues

Hey people! Okay so this is my first post on here so bear with me while I get the hang of things. Let me run this down really quick and get some things out of the way.

• This is an old GPU. I've used my computer just about everyday so this isn't a "new" hardware issue like most of the other posts I read about while troubleshooting this issue.

• I've tried uninstalling drivers, installing new ones, slightly older ones to no avail. I've tried rebooting my PC a dozen times. I've unseated and reseated my GPU twice. I've switched my monitor (for reasons you'll see in the attachments below) because I read that it could be a display port issue. It wasn't. I've tried updating my MOBO drivers, I've tried everything that I could think of.

I've attached any images I feel might be useful, below.

Any help would be massively appreciated.

What gpu?

By the way looks like ram artifacting.
 
In the pictures I posted of GPUz, It recognizes my 780 but nothing about it is labeled. I also posted a photo of my GPU from my device manager saying that it isn't working properly.

Did you buy this board used?

Display problems like yours usually indicate a failing Vram problem.
any overclock on card ? if so remove it
Card is probably dieing.
You could and should test in another system (make sure it has a suitable power supply= or > than yours)
If Fault is replicated then its time to look for a replacment

No OC. Unfortunately, thats what I suspected when I started this endeavor hours ago. I also, unfortunately, don't have another PC to test on. I'm on my last leg with troubleshooting and going to try one last thing. I will mention that the artifacting stopped when I ran DDU and had 0 drivers on my computer and came back once I reinstalled the drivers so thats probably not a great sign haha.
 
Bake it and try it again once it's cooled and assembled.
