Issue with artifacts on my monitor.

My display is showing moving red dots/diagonal lines and squares that come and go on when playing some games. It only happens when I’m in a game playing and it only happens in dark areas. Works perfectly fine for everything else. Also my pc is connected through hdmi and I already tried switching them out. My tv has a refresh rate of 60hz and native 4K. Could it be because I play with vsync off and reach frames as high 120 FPS. Please help. What I’m talking about
 
Replace the dvi/hdmi cable.

Edit: just read you've tried that. Can you try underclocking your video memory?

It's not because you're pushing 120FPS. It's an issue either with the TV connection or (in some cases) the videocard artifacting. Im not sure what card you have but those look like artifacts you would get with unstable vram.
 
Is the 60Hz adjustable?
Try 59Hz just to humour me.
