Is my card dying?

So for the past month or so I would say I have started to have an issue. Every once in a while my screen will go black and my computer will make the noise like a device was unplugged. Then my screen will come back on and in the bottom corner a message pops up saying that Nvidia Driver blah blah blah stopped responding and has recovered from an error. Or something very similar to this. My card is fully updated and I purchased it probably a year or so ago. Haven't done any super intense gaming on it.

Card: Gigabyte Nvidia Geforce GTX 680.

Something else I have noticed (not sure if this is the game, or again my card) when I first start up Rocket League, the first goal that is scored in the very first game I play, the game will hang for 3-5 seconds and then pick up and be fine from there. It doesn't happen after this. Also, yesterday when I closed Rocket League and maximized Firefox again the card crapped out and restarted.

This issue has been going on like I said, for about a month now. It only happens once a week or so, if that.

Is my card trying to die?

when your screen "craps out" are You gaming, or is the GPU under heavy load/strain? or is it just regular dormant GPU, web browsing stuff?
Are You running an Over Clock on your GPU?
Is Your CPU, or RAM OC'd?
Do You have the ability to put the 680 into another functioning machine , and test its stability out of your PC?
Or do You have a second GPU, you can install in place of the 680 to test as a control?

the last 2 questions are where i would start, then removing ANY over clocking that might be on Your GPU, and CPU, as well as running default RAM timing's. Also, running in safe mode might help.

I suggest instead of trying to figure out the problem, try figuring out what the circumstances are that are causing it, It will likely be easier than the former , and will certainly help in finding the solution.
MY guess, with limited information , is it is NOT the GPU dying, but that is My 2 cents @ a glance.

That symptoms seem like the card is either overheating or its having a divorce affair with its drivers.

Does OC come into play here?

When my screen goes black it generally happens when not doing anything to graphic intensive. Like last night.

Was playing Rocket League
Closed the Game
Opened Firefox and went to Youtube. Started to play the video and then screen went black, then came back with the error message and Firefox had closed itself.

The only thing that is OC'd is the CPU. Instead of running at 3.1 it is set to 3.4. However, this has been the case since before I even had this card in there. So, this card has been running with the CPU OC'd for the past year or so now problem free. I don't have another machine I can drop this card in and sadly, no other card to drop in my machine to test that way either. I ran Furmark earlier and had no issues. Although, I only ran it for maybe 5 minutes.

My 580 likes to crap out on me every once in a while, it's caused by dust building up and clogging the cooler. I just unscrew the cooling shroud and blow it out, then it's right as rain again. You should investigate your cooler for dust.

I'll give that a go. I haven't cleaned it since I put it in about a year ago so I am assuming there is at least a little dust in there.

what driver do you have, last time my 980 got a lot of driver recovery it was with the latest drivers at the time (358 or something)
although the latest driver are fine now (none the later ... the last stable driver for me, before that one, where the 347.88 )

what power supply? your specs just say 550W, if its getting hot and voltages are spiking, it could cause this.

Current Driver is 359.06
what power supply? your specs just say 550W, if its getting hot and voltages are spiking, it could cause this.
