File system is corrupted. Wipe and start over. Not much you can do about it since Sony does encryption stuff.

You will have to boot the recovery USB file.

This could be a thing, and maybe secure erase that disk IF it's still alive.

Like the USB recovery boot that is needed to initialize a new drive?
Similar like with PS3? I have no experience with PS4, but at least with PS3 you need to have the firmware on a USB drive to install it with a new drive

What is the exact error code that your console gives out? Look it up here:

The PS4 has a very comprehensive list of exceptions and each one of them has a different root cause. That said, if the issue began after replacing the main storage, it'd be wise to replace it again.

Similar like with PS3? I have no experience with PS4, but at least with PS3 you need to have the firmware on a USB drive to install it with a new drive

On most units that have the 16 MB NOR chip for the bootstrap. The early series PS3s with 256 MB NAND (CECHA through the first non-BC, CECHG) have the firmware installed onto the motherboard itself. You can swap HDDs without a format or losing data. It's insanely good. Even that, Sony took from us :banghead:

PS4 has two firmwares. One for updating the version and one to restore. Sony doesn't make it easy to find.

I would have considered both except the drive replacement was the first thing i did when i got it i knew the disk was faling and validated it with my computer
The error history is all over the place
but the most common codes is
CE-34054-6 which doesn't come up on the website
only recent error thats on the website is "CE-34878-0" which says to "Error has occurred with the application (shows up after a game crashes) // Step 1: Close the application, install the latest system and game updates and reboot the system. Step 2: If you upgraded the HDD, use the original one. Step 3: If the error occurs again, back up all saved data and initialize the PS4. Step 4: If the error occurs with every application or as soon as the application starts, contact PlayStation Support. "
I cannot replace the disk it was failing *The original disk*
Their is also CE-41812-6 AGAIN not on the website
theirs also a ton of codes most that relate to An error has occurred in the following application or a "Can not continue using the application"?
most of which are followed by the database is corrupt

Is it reported as corrupted or just constant database fix operations?

As an example my PS5 fixes its database on every update now. Seems to be a normal thing unless I got unlucky with shipped nand.

it says
"The Database is Corrupted
Please Reboot the PS4"
Yes... that message means you need to reinitialize the database. For PS4 that is booting to the USB recovery and basically factory resetting the console. I get the same message when I put a new drive in. The console will not work until you do.
