USB not recognized problem

95 out of every 100 times my cell phone doesn't work when connected by USB and displays this image.
What could be the reason which makes it work?
What should I be doing to have it working every time?

My guess is the cable.Try connecting the phone to the pc and twist the cable thoughout the lenght to see if it makes difference.
 
check that your USB Chipset drivers are the latest ones for your hardware, or just try plugging it into a USB 2.0 port or force the USB 3.0 into USB 2.0 using a USB 2.0 USB hub.

Ive had this problem with older devices before and the only way to solve it was to force USB 2.0 if nothing at all. some devices just wont work with USB 3 for some reason.
 
Cable or one of the ports on either the phone or the PC
One of my USB sticks does this sometimes as well due to bad contacts on the stick
