NV Control Panel doesn't actually say, do you have to enable Adaptive V-Sync in NV CP and also enable normal V-Sync in a game to make it work or is Adaptive V-Sync fully independent?

I'm asking because when I tried it with it enabled in NV CP and disabled V-Sync in Killing Floor 2, image was still tearing like insane.

It's even more confusing because Fast V-Sync works by only enabling it in NV CP and it just overrides everything, even if V-Sync in games is disabled. So, I'm a bit confused now...

For all the 3D settings, there's a global version and a "local" game-specific version.

If you're using the global version, then make sure that the local version is unset to "use global" otherwise it will be overridden by the local version. Otherwise, just find the game in the driver and fiddle around with the settings for the specific game.

These global and local settings override the settings in the game. I have noticed however, that sometimes a game overrides the driver and this seems to be a bug somewhere, because it usually resolves itself after a couple of revisions. This might be what you're seeing here.
