USB Device Not Recognized (Malfunctioned)

I just got everything in my NZXT Vulcan and for some reason both oh my USB thumb drives aren't recognized, Windows 7 Pro 64-bit says if it doesn't work after replugging then it has malfunctioned and to replace the drive. This is happening on two of my thumb drives that I've verified are fine on different machines. This problem is happening for both the case's front USB ports & the motherboard ports.

I looked up this issue on the web & it says to uninstall all of your USB drivers and let Windows reinstall them, tried it and didn't work. Reset the CMOS, didn't work. Yanked the power supply cord out for about a minute, didn't work. Pretty stumped right now, anyone have any ideas?
 
So you have removed all the USB drivers through the device manager and it is still acting up.

I would remove the internal USB leads to the case mounted USB sockets and see if that makes the ones on the motherboard function better. Make sure you have the latest drivers I had a somewhat similar issue with an old MSI motherboard but that was way back on S478 :P
 
I encountered a problem like this. Switch off your PC and try one USB port at a time. If one doesn't work. Switch off and try the next. Because after that error comes. It sometimes disables all the USB ports. Also you can try holding your thumb drive up. (My thumb drive doesn't work on my friends PC because his USB port is slightly big. So the pins on the thumb drive doesn'tt touch the pins on the USB port. Other than that you can try switching off and switching on your PC without any USB devices plugged in and then try plugging in your thumb drive. (Some time ago i had a HSDPA that would disable every other USB port if i switch on my PC it plugged in. :/ )
 
I had that once when I had a bad rear port expander, it did disable all the other ports, replacing the rear port expander solved the problem.

Tried removing the front panel USB's and restarted, still doesn't work. I'll try finding some motherboard drivers, I just don't understand why everything worked prior to installing the stuff inside the case. Maybe it was just a coincidence?

I've tried all of the USB ports, none of them worked. I tried holding it up and Windows had a message saying it was looking for software drivers for the device and couldn't find any so it wasn't installed properly. I tried rebooting with just a PS/2 keyboard and the thumb drives still didn't work. Mice, keyboard, speakers, and printer works just fine though.

By rear you mean front panel USB ports right? If so, I've removed the plug and it still isn't working.

You guys might be onto something about all USB ports being disabled, I hooked up my phone to the computer via a USB cable and the device wasn't installed properly. I didn't care since I was just charging the phone. This was the same thing that happened moments ago with the thumb drive.
 
You might have pulled a wire loose somewhere or there may be a short.

I had the ground wire on my usb cable for my phone come off inside the jacket and it was doing the same thing it would detect and error but still charge.
So maybe one of the ports has a faulty wire or one got plucked off when hooking everything up, also make sure you attaching the usb header to the motherboard the right way .
 
I'm going to remove the board and see if there's anything wrong. Actually, lets see if the ports will work without the case.
 
Good thinking you might have had a piece of flash or something shorting the pins on the usb's somewhere under the mobo or a bad soider joint somewhere .

No shorting under the motherboard and nothing seems to be in the USB ports. The motherboard is on my desk right now not hooked up to a case. Nothing works! I checked my phone's USB cable and compared it to my mouse's USB plug, they look identical with no missing parts pieces of metal anywhere. I'm really stumped here, no idea why I can't get my thumb drives to work anymore.

If I can't get this fixed in about 4 hours I'm reinstalling Windows, but from what I've read on the web that doesn't work either. I'm going to try downloading the latest USB drivers from EVGA, will be back with an update within the hour.
 
Okay, no luck. Every so often Windows 7 will try and find the right drivers but fail to and say that the device wasn't installed properly. Anymore ideas before I do a complete reinstall?

HOOOLD UP!

Both my thumb drives are currently NOT WORKING ON ANY COMPUTER! WTF!

Edit: System restore didn't work either. Reinstalling in a few moments, thank god I have an SSD and all my files are on two separate hard drives.

Just had this problem recently, a cmos clear corrected it. One item you can check to see is if the bios recognizes anything by loading its "boot from" menu and checking for any USB mass storage listed.

Doubt it, MalwareBytes came out clean.

I've tried resetting the BIOS so many times, I'll check to see if the drive shows up in the BIOS in one moment.

You know what, I connected the front header to the 1394 thing until I double checked last night and fixed it. That wasn't before I connected one thumb drive to the front USB port though... NLOGKJSHFVBSKF

The second one isn't working either and I'm 100% positive I didn't connect that one to any port until after I fixed the header.
